Massive Splenomegaly


Massive Splenomegaly



Definition: Clinically if the enlarged spleen reaches the right iliac fossa  by crossing the midline or if it weighs more than 1.5 Kg we call it as massive splenomegaly.




Common Causes Massive Splenomegaly:

1. Myelofibrosis
2.Chronic Myeloid Leukemia
3.Malarial Spleen (Tropical splenomegaly)
4. Gaucher's disease 

Complications of Massive Splenomegaly:-

1. Rupture of spleen with minor trauma
2. Splenic Abscess formation.

Why spleen enlarges towards the Right Ilica Fossa? 

 Attachment of Phrenico colic ligament prevents the enlargement of spleen in the downward direction in to left iliac fossa.



 What is Harri's Dictum?


The Harris’ dictum of odd numbers 1, 3, 5, 7, 9, 11 summarizes  splenic data - Normal spleen measures 1 inch in thickness, 3 inches in breadth, 5 inches in length, weighs 7 ounces, and is located  in relation with  9, 10, and 11 ribs on left.
